- What steps will I need to take to place my loved one in a
nursing home?
Select a nursing home that you want to place your
loved one in and see if they have a room available.
Contact the Area Agency on Aging and request that
a CARE assessment of your loved one be done to determine his/her
appropriateness to be placed in a nursing home.
- The Assessment is done FREE of charge.
- The Assessment is good for one year.
- The Assessment will be required by almost all nursing homes.
Determine how payment will be made:
- Out of personal financial resources (private pay)
- From long term care insurance
- From State assistance (Medicaid)
(Be sure to ask if the facility of your choice will accept
residents on Medicaid (State assistance.)

- Will Medicare pay for a nursing home stay?
Medicare will not pay for a nursing home stay unless:
- The nursing home is Medicare certified.
- The doctor has ordered the resident to receive skilled services
and these services can be provided by the facility.
NOTE: Through the Progressive Health Care Alliance, Memorial
Home has staff on site to provide skilled therapies-physical,
speech and occupational.

- What financial resources are available if my loved one's
financial resources are limited or non-existent?
For persons with limited or no financial resources, some assistance
is available through the State of Kansas. This assistance will
likely be Medicaid or Home and Community Based Services (HCBS).
To determine whether one qualifies for this assistance and how
one qualifies, contact the SRS (Social and Rehabilitative Services)
office nearest you. This is critical as some assistance requires
personal resources to be spent down to a certain level and other
assistance requires being placed on a waiting list.

- Will I fit in at Memorial Home?
Memorial Home is located in a rural setting and sponsored by nine
local churches, six of which are Mennonite, two independent and
one Methodist. Many of our residents grew up on farms and spent
their lives farming. Many of our residents were born in Moundridge
or other small rural communities and then left the community for
the larger cities and foreign countries. Now in their retirement,
they are coming home. We're also quite ethnic in some of the wonderful
German, Swiss and Russian foods we eat. We welcome diversity of
backgrounds, education, religion and employment. If you join us
we will want to get to know you and to tell you about ourselves.

- What does it mean that Memorial Home is a CCRC?
CCRC stands for Continuing Care Retirement Community. Memorial
Home as a CCRC offers a continuum of housing and services to seniors.
This continuum covers:
Independent Living
- Single homes
- Duplex homes
- Six-plex homes
- Congregate apartments
Assisted Living
Health Care (skilled nursing)
Special Care (dementia unit)
Once in the continuum of housing and services residents will establish
priority as their situation dictates.

- What is meant by levels of care?
In healthcare and assisted living we have established base rates
with levels of care. This is a system that recognizes each resident
does not need the same level of care and allows nursing staff
to continually assess the care required by the resident and is
billed accordingly.

- What is the evidence that Memorial Home is a good placement
for my loved one?
All nursing homes and retirement communities in the State are
surveyed annually. Following are the results of Memorial Home's
surveys for the past two years.
2000 - 2 deficiencies
2001 - 1 deficiency
Our administrator often says the best way to determine good
placement is to go to the facility and tour, noting:
What you see.
What you hear.
What you smell.
The most frequently received compliment is that this place
does not smell like a nursing home. This compliment was passed
on by the State Environmental Specialist when he was recently
in our facility.
